A Release of Information is needed when a client wishes for their information to be released. The counselor is required to keep your personal information private. There are times when a clinician is required to release information. We are required by law to release information if a client becomes a danger to themselves and/or others, in the event of child/elder abuse, or abuse of an incapacitated adult, or if the counselor receives a court order of subpoena. In these cases, the counselor will consult with other professionals without giving identifying information, and will limit the release of information as much as possible to release only what is required by law.

In order for records to be released, a signed Release of Information Form is needed. Exceptions listed above.
